Healthier West Wirral PCN Neighbourhood Engagement

The Neighbourhood Health Engagement Lead will focus on Health Promotion in West Wirral, by establishing a neighbourhood model, linking Non voluntary sector, social care and community assets to develop, deliver, and evaluate collaborative health and wellbeing initiatives, ensuring that the voice of local people is central to neighbourhood-level planning and delivery.

Areas of focus will be developing a sustainable year-round vaccination model, improving early cancer detection through enhanced screening and community engagement.

Alongside this, the neighbourhood health engagement lead will be working to develop a gold standard frailty service, complimenting work to reduce avoidable emergency department admissions through the implementation of personalised multi agency support.
